What is Shock Wave?
MOA: shock waves exert stress in tissue via 2 mechanisms:
- Acoustic waves that carry energy and when propagating through a medium, can be targeted and focused noninvasively to affect a distant selected anatomical region
Direct mechanical stress associated with the high-amplitude shock wave
The growth and violent collapse of cavitation bubbles in fluid (injury within the blood vessels vs surrounding tissue). Smaller vessels (capillaries) will be subject to greater stress than larger vessels causing shear stress and damage of the endothelium resulting in neovascularization (formation of new blood vessels)
- Variety of shock wave therapy exists: Focused LI-ESWT, Non-focused/Linear LI-ESWT, RSWT
- Focused LI-ESWT originally introduced for kidney stones and since been used in the management of bone fractures, musculoskeletal disorders, cardiovascular disease and wound healing
Mechanism of Action
Peak pressure exerts direct mechanical stress
Longer-lasting period of negative pressure generates gas bubbles in fluid (i.e., blood vessels) that violently collapses (cavitation) causing trauma to the endothelium
Smaller vessels are subject to more trauma than larger vessels
Trauma sets off a cascade of wound healing responses that induces neovascularization and angiogenesis (formation of new blood vessels)
